About the Role
Halcyon Home is seeking a Home Health LVN PRN PV to support our growing geriatric home health population in South Austin, TX and surrounding areas. We offer competitive pay and wonderful team support.
Responsibilities
- Under the direction of the RN, assists in identifying the patient’s physical, psychosocial, and environmental needs as evidenced by documentation.
- Expected to be present within our partnering communities and our clients independent residences.
- Provides effective communication to the patient, family, team members, and other healthcare professionals as evidenced by clinical notes, case conferences, and communication notes.
- Completes appropriate documentation in a timely manner.
- Monitors assigned cases to ensure compliance with requirements of third-party payor.
- Participates in the Agency sponsored in-service trainings.
- Promotes the Agency philosophy and administrative policies to ensure quality of care.
- Participates in planning and implementing care in partnership with the patient, representative (if any), caregiver(s), and the RN.
- Provides services that are ordered by the physician as indicated in the plan of care.
- Provides care using infection control measures that protect both staff and patient.
- Provides patient and caregiver education.
- Participates in the Agency’s QAPI program.
- Demonstrates commitment, professional growth, and competency.
- Assures the continuity of care through delivery of quality patient care.
Requirements
- Graduate of an accredited school of vocational/practical nursing.
- Active, state license as a licensed vocational/practical nurse, or a multi-state license issued by a Nurse Licensure.
- One year of experience as a licensed vocational/practical nurse in a clinical care setting required.
- Home health experience, preferred.
- Homecare Homebase platform experience is a PLUS!
- Nursing skills as defined as generally accepted standards of practice.
- Good interpersonal skills.
- Proof of current CPR certification.
- Reliable transportation and valid auto liability insurance.
