About the Role
Provide direct care and support to residents under the supervision of the RN Nurse Manager, ensuring their physical needs and daily living requirements are met according to individual care plans.
Responsibilities
- Provide direct grooming support including bathing, shampooing, oral hygiene, shaving, and nail care.
- Assist with resident mobility, ambulation, and active exercise programs.
- Gather, report, and document health information including vital signs, weight, and functional changes.
- Assist with medication recordkeeping and resident transfers.
- Provide support for incontinence care, oxygen, ostomy, and catheter maintenance.
- Maintain a safe and clean environment by managing linens, trash, and apartment tidiness.
- Communicate pertinent resident information to nursing staff during shift changes.
Requirements
- Certification as a NYS Home Health Aide or Personal Care Aide.
- High school diploma or equivalent.
