About the Role

The Care Manager within the Critical Time Intervention (CTI) program plays a vital role in supporting individuals transitioning from institutional settings into the community. This position ensures continuity of care by providing intensive case management services and working collaboratively with internal CTI teams, hospital discharge teams, and community providers. The role emphasizes proactive coordination of mental health, housing, and developmental disability services while advocating for participant independence and long-term stability.

Responsibilities

  • Provide intensive case management services to individuals with complex needs.
  • Ensure clients have access to mental health, medical, and community resources that support community integration.
  • Coordinate care plans and adjust them according to the evolving needs and goals of each participant.
  • Facilitate smooth transitions by collaborating closely with hospital discharge teams, clinical staff, and community service providers.
  • Conduct field-based outreach and follow-up, ensuring continuity and consistency of care.
  • Support and empower clients to navigate systems independently and advocate for their needs.
  • Participate in rotating on-call responsibilities one week per month.
  • Complete documentation and reporting in alignment with agency standards.
  • Work both independently in the field and collaboratively in a hybrid team office setting.
  • Use agency vehicle or personal vehicle (mileage reimbursed) for client transportation and field visits.

Requirements

  • Strong organizational and time management skills.
  • Effective problem-solving and critical thinking abilities.
  • Capacity to manage and prioritize complex caseloads.
  • Demonstrated ability to work across systems (healthcare, housing, disability services).
  • Empathetic, client-centered approach with strong interpersonal communication skills.
  • Must be capable to access all rooms in a 2-story home.
  • Able to work in open space floor plan.
  • Must be capable to move throughout work day.
  • Occasional lifting of > 25+ pounds.

Skills

  • Spanish language proficiency is a plus.

Experience Level

  • Minimum of one year of experience in a care management or case management setting.
  • Experience working with individuals with mental health needs and/or intellectual and developmental disabilities (I/DD) strongly preferred.

Education Level

  • Bachelor’s degree in social work, psychology, or a related field is required.

Salary/Compensations

  • $26.00 an hour
