About the Role

Provides care management services to program participants in need of assistance with medical, behavioral health, and social services in the community, under the supervision of the Director of Care Management Services Division or designee.

Responsibilities

  • Identifies individuals in the community needing medical, behavioral health, and social services.
  • Contacts assigned participants within 3 days of assignment.
  • Maintains an active caseload of 45+ individuals.
  • Prepares a Care Plan with participant goals for each participant.
  • Obtains necessary medical, behavioral health, and social services for participants.
  • Maintains regular contact with participants, including home visits as needed.
  • Maintains accurate and updated records of all services provided.
  • Attends staff meetings and training sessions.
  • Participates in community activities as a representative of the program when assigned.
  • Is available to work outside regularly scheduled hours as needed.
  • Performs other duties as assigned by supervisor.

Requirements

  • Ability to take care of customer needs while following company procedures.
  • Ability to pay attention to the minute details of a project or task.
  • Ability to work effectively with people regardless of age, gender, race, ethnicity, religion, or job type.
  • Ability to demonstrate professionalism conforming to a set of values and accepted standards.

  • Ability to communicate effectively with others using the spoken word.
  • Ability to communicate in writing clearly and concisely.
  • GED or Associate's Degree in Human Services or related field.
  • A minimum of 2 years of experience working with Latinos connecting them with community services.
  • Bilingual in Spanish and English.
  • Demonstrated ability to work with other service providers.
  • Basic knowledge of Microsoft Word.
  • Must provide own transportation and possess a valid, good-standing NYS driver's license.
  • An equivalent combination of training and experience may be accepted if deemed adequate for the job.
