About the Role

Assist with the monitoring and coordination of healthcare services for individuals with developmental disabilities in community-based homes, working under the direction of an RN.

Responsibilities

  • Assist the RN with coordinating preventive and routine healthcare.
  • Complete RN directives within scope of practice and ensure medical follow-up.
  • Follow up on documents in Medical Communication Book, SharePoint Folders, and EHR.
  • Follow up on lab and diagnostic testing, review, and track results.
  • Complete nursing care documentation under the direct supervision of the site RN.
  • Assist with coordinating medical program needs with site staff, management, providers, and agencies.
  • Assist with ensuring adequate scheduling and completion of health-related exams and appointments.
  • Communicate issues and carry out directives from site RN and Nursing Administration.
  • Participate as an active member of the Interdisciplinary Team.
  • Maintain compliance with State/Federal regulations and agency policies.
  • Maintain communication with Medical Liaison/Team Leader and site RN.
  • Participate in assigned nursing workgroups.
  • Complete AMAP certification.
  • Perform CPR and SCIP.
  • Comply with all agency policies and procedures.
  • Perform other duties as required by Nursing Administration.

Requirements

  • High School Diploma
  • Current CPR certification or must attain within one month of employment.
  • Minimum of one year of experience in a healthcare or human service related field.
  • Driver's License that meets agency policy.
  • Ability to meet physical requirements including standing, walking, sitting, bending, stooping, squatting, kneeling, and climbing.
