About the Role

The Children’s Health Home Care Manager works with youth and their family to develop and maintain a comprehensive plan for accessing and using necessary health services and supports.

Responsibilities

  • Complete Mandated Reporting, HIPAA, Health Home Netsmart, and Trauma Training within the first week of employment.
  • Obtain CANS-NY certification and complete all NYSDOH training through the Health Commerce (HCS) and Universal Assessments Systems (UAS) within the first 30 days of employment.
  • Abide by all agency policies and procedures.
  • Attend required trainings and meetings.
  • Maintain confidentiality of individuals and agency functions.
  • Engage and re-engage individuals to ensure positive outcomes with engagement levels of 85% or higher monthly.
  • Complete intakes per AHI policies and procedures.
  • Be in compliance with all tasks as outlined in AHI policies and procedures.
  • Provide mental health education to restore and support environmental stability and avoid restrictive placements.
  • Establish support networks and develop coping strategies for effective functioning.
  • Teach and strengthen daily living skills to assist with self-management and reduction of challenges.
  • Identify personal interests and hobbies within a youth-centered approach.
  • Assess ongoing progress toward recovery and functional skill development.

  • Must be detail-oriented.
  • Must be able to clear NYS Criminal Justice Background Check, Office of Children and Families Background Check, Department of Health Background Check, and clear the NYS staff exclusion list.
  • Must be fingerprinted for the School District.
  • Must clear OIG and HHS-OIG.
  • Ability to maintain a flexible schedule with availability to work evenings and/or weekends as needed.
  • Willingness to learn and self-improve.
  • Willingness to travel 50% or more per week.
  • Must have and maintain reliable transportation including a valid driver’s license, registration, inspection, and insurance.

  • NYS Teacher’s Certificate for which a bachelor’s degree is required
  • NYS licensure & registration as a Registered Nurse & a bachelor’s degree
  • Master’s Degree in any of the above programs
