About the Role
Seeking a Board Eligible/Board Certified Family Medicine or Internal Medicine physician to provide comprehensive primary care services to adult (and optionally pediatric) patients. This role emphasizes clinical independence, efficient workflows, and meaningful patient relationships over volume-driven care.
Responsibilities
- Provide comprehensive outpatient primary care, including preventive, acute, and chronic disease management.
- Diagnose and treat a wide range of conditions across the primary care spectrum.
- Manage complex, multi-morbidity patients with an emphasis on continuity of care.
- Perform routine office-based procedures (e.g., joint injections, skin procedures, basic wound care).
- Coordinate care with specialists, imaging, and ancillary services.
- Utilize EMR for documentation, order entry, and care coordination.
- Participate in quality improvement and clinical initiatives.
- Maintain accurate, timely documentation.
Requirements
- BC/BE Family Medicine or Internal Medicine Physician
- MD/DO license to practice in NY
- DEA License (or eligibility)
- Interest in preventive medicine, wellness, or longitudinal care models.
- Procedural skills (e.g., injections, minor dermatologic procedures) preferably.

- $200,000 - $250,000 per year plus productivity bonuses
