About the Role

The Case Manager provides direct physical and supportive services to young people in Covenant House New York’s short-term emergency Youth Shelter, utilizing an integrated, multidisciplinary team approach. Reporting to the Program Coordinator, the Case Manager will support a culture that achieves meaningful results for the young people served.

Responsibilities

  • Provides integrated counseling and case management services to engage young adults and promote personal growth, wellness, housing stability, and service plan goal achievement.
  • Collaborates with youth to assess strengths and challenges, developing individual service plans reflecting progress and stated goals.
  • Meets regularly with youth to determine progress with goals identified in the Individual Service Plan (ISP).
  • Advocates on behalf of youth with outside service providers and other collateral services.
  • Creates and maintains confidential records, charts, and logs for youth, documenting all relevant information from interventions and collateral services.
  • Ensures youth files are compliant with all regulatory and other requirements.
  • Completes incident reports per agency policies.
  • De-escalates crises and attends to emergencies following agency protocol.

Requirements

  • Bachelor’s degree preferred.
  • High School or GED required with one year of experience in a residential setting working with adolescents preferred.
  • Strong skills in leading groups and collaborating with internal and external departments.
  • Demonstrated knowledge of and experience working directly with young people through resiliency, strength-based youth development, and trauma-informed care approaches.
  • Strong multi-tasking skills with demonstrated ability to manage multiple projects simultaneously.
  • Excellent verbal, written, and problem-solving abilities with an upbeat demeanor.
  • Strong focus on child protection and ensuring resident safety.
  • Experience working with diverse groups.
  • Overall flexibility in work schedule, including weekends, evenings, and holidays if needed.
  • Experience utilizing evidence-based and other social service models preferred.
