About the Role

The MST Coach is a specialized Case Planner role focused on the MST-FIT model of service, supporting youth and families by wrapping services to ensure successful discharge. This role involves performing case management responsibilities using the MST approach and adhering to all relevant regulations.

Responsibilities

• Perform case management responsibility to the youth using the MST approach.
• Perform all activities and administrative tasks associated with handling a caseload in keeping with all ACS and OCFS regulations.
• Conduct field work that sometimes requires driving residents in your vehicle.
• Work occasional evening hours for parent conferences and home visits.
• Demonstrate fidelity to the agency’s service values and organizational imperatives.
• Provide aftercare support to cases transitioning to trial discharge, using MST-FIT model of treatment/ therapy.
• Provide additional support to cases transitioning from trial discharge MST-FIT to return to placement, as necessary.
• Provide crisis intervention and conflict resolution to youths and their families throughout the week, to include evenings/weekends as required, but minimally on a consistent rotation schedule.
• Conduct virtual and in-person sessions to resident youth/families.
• Responsible for at least weekly documentation, including case, collateral, and other contacts within 48 hours of occurrence.
• Ensure ongoing support and collaboration across MST and ITM program/ staff on behalf of residents.
• Prepare and submit MST-FIT engagement court reports for scheduled permanency hearings.
• Prepare for and attend weekly and other ITM learning community meetings and trainings.
• Attend monthly/ mandatory trainings, including 7-day ITM and 5-day MST-FIT booster trainings.
• Prepare for and attend weekly supervision and MST-FIT group consultations as scheduled.
• Prepare for and attend monthly MST-FIT psychiatric consultation as scheduled.
• Support the social service department with regard to resident transition by way of MST-FIT model of treatment engagement.

Requirements

• Minimum MSW degree with licensure preferred.
• Minimum of 1 year of experience working in child welfare with at-risk youth.
• Former Case Management experience is a plus.
• Valid NYS Driver's License.
• Professional, mature, and dependable with a bias for action.
• Ability to assess and engage youth and families is critical to the position.
• Physical ability to travel throughout the community, securing resources and services, while adhering to the MST 120-day targeted length of treatment model.
• Willingness to adjust schedule as needed to ensure service needs are met.
