About the Role
Fred Victor is seeking a 24-Hour Services Mental Health and Addictions Case Manager for the Women's Hostel. This role is crucial in supporting women experiencing poverty and homelessness, aiming to improve their health, income, and housing stability. The position operates within a low-barrier, trans-inclusive, and welcoming environment, utilizing principles of Housing-First, Harm-Reduction, and recovery.
Responsibilities
- Implement harm reduction, client-centered recovery philosophy and community service delivery.
- Conduct community outreach and advocacy to establish partnerships for connecting clients to resources.
- Foster engagement with harm reduction programs, including workshops and social activities.
- Manage harm reduction kit ordering, preparation, storage, and distribution.
- Ensure safe disposal of used harm reduction equipment according to established protocols.
- Provide overdose prevention education and support.
- Engage clients with monthly programs and document meeting minutes.
- Collaborate with the Harm Reduction Committee, clients, and peers to develop harm reduction programming and services.
- Ensure the overall mental health and well-being of clients.
- Liaise clients with community resources for mental health and addiction services.
- Assist clients with symptom management through knowledge of mental illness and addictions on a one-on-one basis.
- Implement trauma-informed practices by ensuring client safety and awareness of potential triggers.
- Collaborate in creating a predictable environment to help clients rebuild efficacy and personal control.
- Utilize a strengths-based approach focused on skills building to enhance resiliency.
- Follow up with clients weekly or as needed based on priority and urgency.
- Review and adjust service plan goals collaboratively with clients.
- Record client notes, intake forms, and discharge plans in accordance with standards and policies.
- Report all accidents, injuries, and occurrences in writing.
- Collect data on client demographics, service statistics, and goal achievement.
- Provide referrals, information, and advocacy to support clients in meeting case plan goals.
- Provide staff back-up for agency programs and daily activities as needed.
- Cooperate with other staff and clients for efficient and coordinated services.
- Process client intakes and transfers, and support clients with daily needs.
- Resolve client complaints appropriately and in a timely manner.
- Lead and support external agency staff providing services onsite.
- Participate alongside individuals to accomplish tasks and develop meaningful activities.
- Conduct crisis interventions, de-escalate situations, practice conflict resolution, and facilitate groups.
- Communicate and demonstrate professional boundaries with clients, staff, and partners.
- Abide by all Fred Victor Centre policies and procedures.
- Work in accordance with the Ontario Occupational Health and Safety Act.
- Participate in regular meetings and evaluations with management.
- Observe work requirements such as punctuality, meeting deadlines, and working with or without direct support.
- Perform other duties as required.
Requirements
- Educational background in social services with a mental health and harm reduction focus, including continuing education for working with vulnerable individuals.
- Minimum 3 years of experience working with individuals experiencing serious mental health/substance use issues, violence, and social isolation.
- Knowledge and prior work experience in the supportive housing and homelessness sector.
- Empathetic understanding of issues facing marginalized women impacted by poverty, social isolation, mental illness, abuse, trauma, and/or substance abuse/misuse.
- Knowledge of substance use and addictions, specifically injection drug use, through lived experience, work experience, and/or education/training.
- Knowledgeable in harm reduction principles.
- Proven experience with overdose prevention, response, and training/experience with Naloxone administration.
- Experience in providing life skills coaching with a commitment to mental health recovery principles.
- Ability to build partnerships and alliances with community organizations and health centers.
- Knowledge of community-based resources and services.
- Experience supporting individuals to navigate service systems across housing, legal, income support, health, and settlement sectors.
- Awareness of and sensitivity to the health issues of a diverse community.
- Experience working with disenfranchised populations.
- Commitment to community development practices.
- Understanding of systems that cause and support homelessness, poverty, and discrimination.
- Understanding of strategies that promote asset development and self-empowerment.
- Ability to develop effective relationships with participants, co-workers, and external agencies.
- Ability to define roles and implement multi-disciplinary team practices.
- Strong skills in crisis intervention and conflict resolution.
- Ability to utilize sound problem-solving and decision-making skills under pressure.
- Ability to create and implement creative solutions.
- Must be available for shift work.
- Experienced with data management, recordkeeping, and computer literacy.
- Strong communication skills, both written and oral.
- Administrative and organizational skills.
- Literacy in Word, Excel, Outlook, Windows, and SMIS.
- A Police Record Check is required by the final candidate prior to hiring.

Salary/Compensations
- $29.40 per hour
