About the Role
EAC Network is seeking a dedicated Liaison to join the Rikers Island Community Re-entry Assistance Network (CRAN). This role focuses on engaging clients prior to release to ensure effective service delivery by CRAN staff, assisting with screening, treatment planning, and case management for clients with co-occurring substance abuse and mental health issues.
Responsibilities
- Meet with incarcerated clients with serious mental illness who have been difficult to engage.
- Explain the CRAN program and engage clients to increase likelihood of enrollment and successful engagement.
- Collaborate with discharge planners on Rikers Island and community-based CRAN Case Managers.
- Complete in-person screenings for incarcerated clients prior to release.
- Hold groups, small meetings, or individual sessions with clients at Rikers facilities.
- Refer clients to mental health, drug abuse, or alcoholism facilities, and arrange transportation if necessary.
- Maintain case documentation, including progress notes and other relevant information.
- Investigate and recommend supportive services for clients.
Requirements
- Bachelors degree in Forensic Psychology, Psychology, Criminal Justice, or behavioral science.
- At least one year of experience with the justice-involved population.
- Good communication skills, both oral and written.
- Ability to work cooperatively with treatment programs, Social Work Department, Department of Corrections, and criminal justice systems.
- Proficiency in computer applications such as spreadsheets, Microsoft Office, calendar, email, and/or database software.
