About the Role
In this role, you will oversee the comprehensive health and well-being of patients within a high-quality, collaborative, team-based architecture. Physicians will participate in regular huddles to proactively identify health care issues and orchestrate a plan of care with team assistance and organizational support.
Responsibilities
- Establish and maintain long-term relationships with patients
- Manage chronic and acute illness and injuries
- Provide well checks and house calls
- Be available after-hours by phone for patient communication and care
- Track and review normal physical development as well as psychological well-being
- Establish outstanding relationships with and lead a team comprised of a Registered Nurse and Patient Care Administrator
- Create personalized reports for patients to help them and the team navigate their health goals and care
Requirements
- Graduate of four-year accredited medical school
- Complete residency in Internal Medicine, Family Medicine, or Medicine-Pediatrics program
- Minimum of five (5) years as a licensed M.D.
- Work experience includes longitudinal primary care
- Trained or worked in an academic medical center
- Ability to work effectively with and through teams
- Licensed MD in the state of California
- Current BLS certification
Skills
- Outstanding written and verbal communication
- Advanced computer literacy, primarily with iOS
- Experience with EHR-based medical record keeping
