About the Role
VHA Home HealthCare (VHA) is seeking Registered Practical Nurses (RPNs) to provide and coordinate home health nursing services. This role offers flexibility and the opportunity to make a significant difference in clients' lives by helping them live independently at home.
Responsibilities
- Assess, monitor, and coordinate care planning.
- Evaluate care plan interventions through ongoing reassessment and evaluation.
- Ensure quality of care by identifying and acting on factors affecting practice autonomy.
- Demonstrate professionalism, leadership, judgment, and accountability in interactions with clients and families.
- Support client safety through continuous quality improvement.
- Inform nursing supervisor of client activities, concerns, or changes in the client or care plan.
- Submit appropriate documentation to the case coordinator.
- Maintain documentation standards according to the College of Nurses of Ontario.
Requirements
- Registered and in good standing with the College of Nurses of Ontario (CNO).
- RPN diploma/degree from a recognized university or college.
- Valid First Aid or CPR Certification.
- Demonstrate effective communication (written and verbal) and documentation skills.
- Demonstrate reliable attendance, punctuality, and availability.
- Commitment to lifelong learning, professional growth, and critical thinking skills.
- A valid Ontario driver’s license (G License) and a reliable vehicle for traveling is required.
