About the Role

FOX Rehabilitation is seeking passionate, licensed Physical Therapists to deliver exceptional one-on-one care to older adults in their homes or senior living communities on Long Island, NY. This role focuses on improving patient mobility, independence, and quality of life through the Geriatric House Calls™ model, offering flexibility and autonomy in managing treatment schedules.

Responsibilities

• Provide one-on-one physical therapy to older adults in private residences or senior living communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions supporting patient wellness, longevity, mobility, and functional independence.
• Document effectively within FOX’s EMR system.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
• Provide telehealth / virtual care when appropriate.

Requirements

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.
• Ability to meet physical demands of in-home and community-based care, including lifting, patient transfers, standing/walking for extended periods, climbing stairs, and performing therapeutic activities.
• Ability to drive to patient homes and senior living communities.
• Ability to wear PPE when required.

Salary/Compensations

• Full-Time: $75,000 - $115,000 annually
• Part-Time and FOX Flex (PRN) will be pro-rated.
