About the Role

Community Housing Innovations is seeking a full-time Case Manager to support program participants in achieving personal and economic independence, obtaining and maintaining permanent housing, and ensuring living units are well-maintained.

Responsibilities

  • Enforce program regulations, policies, and procedures.
  • Ensure clients receive quality services in accordance with program requirements.
  • Assist clients in developing and evaluating Independent Living Plans (ILP).
  • Provide counseling, crisis intervention, referrals, and advocacy services.
  • Develop and maintain a resource network for client referrals.
  • Collaborate with DSS and other service providers to meet program goals.
  • Assist clients with budgeting, resume building, and ADL skills.
  • Conduct face-to-face meetings with clients.
  • Perform regular unit inspections to assess ADL needs and program compliance.
  • Monitor and document the use of agency resources and unit conditions.
  • Report physical plant needs to supervisors.
  • Complete client discharge plans.
  • Submit timely and accurate internal and external reports.
  • Maintain orderly client charts and files.

Requirements

  • Bachelor’s degree in human services or a related field; Master’s degree in Social Work preferred.
  • Extensive experience may be substituted for educational requirements.
  • Two years of experience in a related field such as homelessness or housing preferred.
  • Excellent verbal and written communication skills.
  • Proficiency in English; bilingual in Spanish is a plus.
  • Basic competency in MS Windows, MS Office, and internet usage.
  • Ability to formulate simple financial budgets.
  • Ability to tour property, walk distances, and climb stairs.
  • Strong reasoning, decision-making, and judgment skills.
  • Ability to relate to underserved, homeless, and disabled populations.
  • Ability to work independently and as part of a team.
  • Highly organized with strong attention to detail.
  • Ability to work under pressure, meet deadlines, and multitask.

Salary/Compensations

  • $24.04 - $26.45 per hour
  • $50,000 - $55,000 annually
