About the Role

The Social Worker is responsible for delivering and implementing direct clinical services as a member of the treatment team, providing care to individuals with substance use, co-occurring, and mental health disorders.

Responsibilities

  • Facilitate individual, group, and specialty group sessions professionally and ethically.
  • Manage a caseload of 20-30 clients from entry to discharge, ensuring appropriate care levels and interventions.
  • Provide backup coverage for clinical staff absences.
  • Demonstrate initiative in task completion and efficient time management.
  • Address program issues with the Program Coordinator related to strategic objectives and service delivery.
  • Participate in agency quality improvement programs and support data-driven care enhancements.
  • Plan and implement activities under the direction of the Program Coordinator.
  • Complete all documentation, including progress notes, treatment plans, and reports, in a timely manner.
  • Maintain an organized system for filing and record-keeping.
  • Inform supervisors of any issues or potential deadline challenges.
  • Actively participate in all meetings, utilizing problem-solving skills and providing feedback.
  • Develop and maintain positive relationships with referents and collateral agencies.
  • Maintain communication and teamwork with Evening Program staff and case managers.
  • Submit weekly schedules and obtain prior approval for offsite activities.
  • Maintain appropriate computer skills for job performance, including proficiency with email and Word.
  • Exercise due diligence in delivering quality care in line with the agency's Medicaid Compliance Plan.
  • Communicate any violations of the Medicaid compliance plan or personnel policies to the compliance officer or supervisory structure.

Requirements

  • LMSW or higher preferred.
  • 1 year of experience working with clients with substance abuse disorders.
  • 1 year of experience working with adolescents with co-occurring and mental health problems.
  • 1 year of experience working in an OASAS funded facility is a plus.
  • Commitment to providing quality services and outcome-driven performance measures.
  • Excellent organizational management skills with the ability to coach staff, manage, and develop high-performance clinicians, and set and achieve program objectives.
  • Strong networking and public relations skills with the ability to engage diverse stakeholders, populations, and cultures.
  • Action-oriented, adaptable, and innovative approach to program planning.
  • Ability to work effectively in collaboration with diverse groups of people.
