About the Role

The Licensed Practical Nurse (LPN) provides support to patients and providers in conjunction with Registered Nurses (RN) under the supervision of the practice.

Responsibilities

  • Collect clinical data on patients and report abnormalities to medical providers.
  • Field patient phone calls and troubleshoot medication management issues.
  • Process medication renewals and complete prior authorizations.
  • Document nursing observations, interventions, and patient responses in records.
  • Ensure patients receive timely care and facilitate internal and external referrals.
  • Maintain standard precautions in patient care.
  • Answer phones and document clear messages in EMR.
  • Maintain records of laboratory work and process diagnostic requisitions.
  • Adhere to policies regarding patient rights and confidentiality.
  • Follow protocols for patient message workflow.

Requirements

  • Graduate of an accredited school of Practical Nursing.
  • Currently registered in New York State as a Licensed Practical Nurse (LPN).
  • Excellent communication skills.
  • Minimum of six months experience in a high-volume ambulatory care setting.
  • Ability to communicate effectively with patients, staff, and providers.
  • Ability to move about the office to assist patients and staff.

Salary/Compensations

  • $28.62 - $35.74
