About the Role
The Licensed Practical Nurse (LPN) provides support to patients and providers in conjunction with Registered Nurses (RN) under the supervision of the practice.
Responsibilities
- Collect clinical data on patients and report abnormalities to medical providers.
- Field patient phone calls and troubleshoot medication management issues.
- Process medication renewals and complete prior authorizations.
- Document nursing observations, interventions, and patient responses in records.
- Ensure patients receive timely care and facilitate internal and external referrals.
- Maintain standard precautions in patient care.
- Answer phones and document clear messages in EMR.
- Maintain records of laboratory work and process diagnostic requisitions.
- Adhere to policies regarding patient rights and confidentiality.
- Follow protocols for patient message workflow.
Requirements
- Graduate of an accredited school of Practical Nursing.
- Currently registered in New York State as a Licensed Practical Nurse (LPN).
- Excellent communication skills.
- Minimum of six months experience in a high-volume ambulatory care setting.
- Ability to communicate effectively with patients, staff, and providers.
- Ability to move about the office to assist patients and staff.
