About the Role
Meadow View Nursing Center is seeking an experienced Registered Nurse Assessment Coordinator (RNAC) to support regulatory compliance, resident-centered care, and survey readiness within their nursing and rehabilitation program.
Responsibilities
- Conduct and coordinate the development and accurate completion of the resident assessment and care plan process in compliance with federal, state, and local guidelines.
- Coordinate scheduling of OBRA and Medicare/managed care assessments, entry/re-entry records, and care plan reviews according to regulatory guidelines.
- Inform residents, responsible parties, and staff of Medicare/managed care coverage status through appropriate documentation and updates.
- Work with Community Health Choice (CHC) case managers for Managed Medicaid providers.
- Participate in committees as directed.
Requirements
- Associate or Bachelor Degree in Nursing preferred.
- Previous experience in completion of Resident Minimum Data Sets and Care planning preferred.
- Basic computer knowledge is essential.
- Current RN license is required.
