About the Role
The Nurse Practitioner will report to the Medical Director or supervisory Attending Physician, and is responsible for delivering compassionate and quality medical care to assigned residents in coordination with the health care team.
Responsibilities
- Examine residents and deliver quality care in varied settings including subacute, long term, rehab, and outpatient care.
- Assist in resident/patient/family education.
- Re-examine, re-evaluate, and modify care plans as necessary.
- Maintain proper documentation in resident charts.
Requirements
- Two (2) or more years of experience in a sub-acute or long term care facility.
- Graduate of an accredited nurse practitioner program.
- Active license as an NP in New York State, with prescriptive privileges.
- NYS Board Certified.
- Proficient in SigmaCare or another EMR (Electronic Medical Record) software.
- Prior experience with the geriatric population is a plus.
