About the Role
The Housing Preparation and Support Worker assesses and identifies clients for long-term housing through counseling, referral, and outreach. The role involves providing ongoing support and follow-up services to ensure clients' long-term success in their housing.
Responsibilities
- Implement housing preparation plans with clients, including ID, taxes, and income supports.
- Provide crisis counseling and emotional support to proposed residents.
- Refer clients to appropriate services.
- Assess and identify potential housing needs and readiness for permanent housing in collaboration with staff.
- Assess and triage clients to the appropriate level of housing support.
- Work with clients to identify housing support needs and delivery methods.
- Assist clients with Social Housing wait lists (TAWL, Access Point) and update applications.
- Develop relationships with landlords to facilitate housing access.
- Engage in individualized housing searches with clients.
- Facilitate housing preparation to help clients become potential tenants.
- Arrange steps for clients to move into housing, including income verification and financial arrangements.
- Arrange and maintain regular home visits with tenants.
- Support tenants in maintaining their housing.
- Make referrals to appropriate services.
- Accompany clients to appointments as needed.
- Liaise between property managers and tenants.
- Advocate on behalf of tenants.
- Educate tenants on their responsibilities and rights.
- Hold house meetings as required.
- Case conference with hospitals, doctors, and other support agencies.
- Re-assess housing support needs as required.
- Assist clients with interpersonal skills, self-reliance, community building, and other skills for housing maintenance.
- Plan and implement special events for housing program residents.
- Share responsibility for office staffing within the Homeless, Hostels and Housing department.
- Provide individual support and counseling.
- Mediate conflicts between clients and surrounding individuals.
- Maintain appropriate information for statistics and files.
- Maintain client files.
- Attend department team and agency staff meetings.
- Provide letters for clients.
- Escort clients to hospitals, doctors, court, etc.
- Arrange funerals, notify families, and liaise with police as required.
- Perform other assigned duties.
Requirements
- One year or more of previous experience working in a shelter and/or housing environment.
- One year or more of previous experience assisting clients access and maintain housing.
- Practical knowledge of crisis intervention, conflict resolution, counseling, harm reduction, community service agencies, and the homeless population.
- Demonstrated ability to use sound judgment and perform job duties with minimal supervision.
- Demonstrated ability to problem-solve.
- Demonstrated ability to work sensitively with diverse individuals and groups in a community setting.
- Ability to collaborate with individuals, client groups, agencies, and organizations.
- Knowledge of political, structural, and sociological reasons for homelessness, under housing, and poverty.
- Ability to quickly identify issues and possible solutions.
- Strong written and verbal English language skills.
- Excellent writing skills for social assistance appeals and letters.
- Ability to work with professionals in community agencies and government offices.
- Experience working with seniors.
- Ability to work as an individual and as a team member.
- Must be legally qualified to work in Canada.
- Able to provide a Vulnerable Sector Screening check with a 'Clear' or satisfactory result.
