About the Role
The Health Coach (HC) is responsible for a panel of patients. In collaboration with the care team, the HC helps patients work on their health-related SMART goals. This role is rewarding as it directly affects patient care, builds trusting relationships with patients, and impacts health outcomes. Responsibilities
- Call patients between visits to discuss their care plan, educate them on their disease states, uncover barriers patients have in following their treatment plans, and assist with medication adherence.
- Enroll patients into the health coaching program by explaining the benefits of the program and how you can help patients as a part of their health care team.
- Educate patients on diabetes, hypertension, asthma, obesity, hyperlipidemia, and gaps in care using Santovia and practice approved patient education materials.
- Provide patients with clear instructions and education utilizing appropriate guidelines.
- Establish SMART goals through motivational interviewing.
- Create care plans for patients with chronic medical conditions to improve quality patient care and reduce hospital readmissions.
- Document care plans in the EMR using the care planning module in eclinical works.
- Document data in spreadsheets by adding important information about patients status in the program.
- Track patients progress and adjust goals as needed.
- Collaborate with the patients care team by having monthly meetings with providers.
- Communicate with physician, Nurse Practitioner, or other health care providers regarding patient’s status in telephone encounters assigned to the assigned provider.
- Direct patient to keep follow-up appointments with physician office/specialists and to maintain self-home care based on physician treatment plan.
- Participate in related training by attending health coach webinars and 1 on 1's.
- Report to the management team on the daily numbers reports and monthly care plan goals.
Requirements
- 5 years in a similar role for Medical Assistants
- 1 year for LPN's
- 0 years' experience if you have completed a Medical Doctorate Degree - Worldwide
- High school or equivalent (Required)
- 1 year of computer skills (Required)
- Spanish language proficiency (Required)
- Medical Assistant, LPN, or higher training (Required)
