About the Role

Responsibilities

  • Conduct comprehensive assessments and re-assessments of residents, including psychosocial, physical, mental health, environmental, and spiritual needs.
  • Write comprehensive assessments and develop client-centered Community Living Plans with clients.
  • Identify, arrange, and monitor appropriate community services, leveraging knowledge of Medicare, Medi-Cal, and other entitlement programs.
  • Establish and maintain care management relationships with clients and their significant others, providing crisis intervention, advocacy, problem-solving, and therapeutic support.
  • Meet with clients at least monthly to review and modify their Community Living Plans.
  • Document all case management activities and identified problems via progress notes within 24 hours.
  • Maintain required paperwork and a consistent charting system for continuity of care.
  • Educate clients and significant others about resources and train them in self-case management.
  • Establish and maintain open communication with community providers, sharing client care information while maintaining confidentiality.
  • Monitor the quality and quantity of services provided by other involved providers.
  • Collaborate with clients, caregivers, and services to terminate clients when appropriate, documenting the process.
  • Participate in research studies and data collection as required.
  • Participate in and promote Continuous Quality Improvement efforts.
  • Attend and actively participate in team and program meetings, contributing to open communication.
  • Utilize supervision appropriately, maintaining open communication and providing caseload updates.
  • Incorporate ethical standards of the National Association of Social Workers into all interactions.
  • Understand and apply regulatory and procedural requirements of Community Living Services and Institute on Aging policies.
  • Attend continuing education and in-service training to enhance knowledge and skills in case management, gerontology, and related areas.
  • Perform other reasonably related responsibilities as assigned.

Requirements

  • One year of experience working with disabled adults and/or frail elderly.
  • Experience with and understanding of the medical and psychosocial problems of functionally impaired adults and the frail elderly.
  • Exceptional communication and presentation skills relating to the frail elderly, functionally impaired adults, their support systems, and teams of health professionals.
  • Demonstrates case management skills and experience in the community health care delivery system.
  • Detail-oriented with good problem-solving skills and the ability to prioritize multiple tasks.
  • Computer literacy required.
  • Ability to lift up to 25 lbs.
  • Ability to reach, bend, and walk.
  • Five finger manipulation.
  • Ability to speak and write English clearly.
