About the Role

The Lead Social Worker provides case management, supportive counseling, and crisis intervention for older adults aged 62 and older at the Park 79 affordable housing location. This role focuses on promoting independent living, community engagement, and wellness through comprehensive social services and collaboration with property management.

Responsibilities

  • Provide conflict resolution and address tenant concerns requiring enhanced intervention.
  • Prepare and submit monthly, quarterly, and ad-hoc reports for funders and the organization.
  • Oversee staff scheduling to ensure consistent presence at the residence during business hours.
  • Collaborate with the Property Management team to resolve facility and tenant-related issues.
  • Conduct intake and assessment screenings for mental and physical health needs and benefit eligibility.
  • Perform home, office, and community visits to address unmet client needs.
  • Implement short and long-term interventions regarding health, housing maintenance, and benefits.
  • Develop community-building events to facilitate socialization and wellness.
  • Lead support groups providing emotional support and psychoeducation on aging and self-care.
  • Maintain accurate and timely client files and progress notes.
  • Perform additional related tasks as assigned.

Requirements

  • Master’s degree in social work or related field, or at least four years of experience in the human services sector.
  • LMSW or LCSW strongly preferred.
  • Minimum of two years of relevant experience in social services.
  • Experience working with older adult populations and families preferred.
  • Bilingual fluency in Chinese or Spanish preferred.
  • Proficiency in Microsoft Office.
  • Ability to provide accurate and timely documentation.
  • Strong time management skills.
  • Ability to work independently and collaboratively in home and office environments.
  • Commitment to maintaining client and agency confidentiality.
  • Excellent interpersonal, public speaking, and written communication skills.
