About the Role

The Licensed Practical Nurse (LPN) will function as a member of the Integrated Health Care Clinical Team, assisting with a full range of nursing and medical services for clients. The LPN will perform tasks and responsibilities within the framework of health teaching and supportive/restorative care, under the direction of a registered professional nurse.

Responsibilities

  • Performs on-site visits and consultations with clients and staff to determine current health status and any intervention needed.
  • Provides education and training to clients and staff on health-related topics.
  • Provides health monitoring and screening to clients, including phlebotomy procedures.
  • Provides medical crisis intervention as needed in consultation with appropriate personnel.
  • Ensures adequate provision and documentation of health services provided at the clinic site.
  • Assists in providing medication refills and prior authorizations.
  • Assists in acquiring updated medical health information from approved healthcare providers.
  • Consults with families as appropriate to ensure communication of pertinent health-related information.
  • Collects information for reviews as directed.
  • Assists in maintaining accurate communication and transfer of health information with health providers and other service providers.
  • Maintains and updates required records and documents for services provided.
  • Monitors and instructs staff/clients regarding preventative healthcare and other health-related issues.
  • Handles emergency situations which may require flexibility of time and areas of assignment.
  • Performs other duties, responsibilities, and related tasks as assigned.

Requirements

  • Graduate of an accredited LPN program with current licensure in NYS.
  • One year of clinical experience as an LPN.
  • Experience working with the mental health population preferred.
  • Must have eligibility for full and unconditional participation in the Medicaid and Medicare programs.
  • Excellent verbal and written communication skills.
  • Ability to effectively use an electronic medical record for all documentation.
  • Must have a valid New York State driver's license.
  • Must have access to reliable transportation when required.
