About the Role

The Care Partner is a support care provider who functions under the direction of the Registered Nurse, focusing on promoting a positive patient experience by assisting with daily living activities and comfort measures. The role involves partnering with the healthcare team to provide excellent patient care and may require advanced technical skills.

Responsibilities

  • Assist with daily living activities and comfort measures.
  • Perform basic patient care functions such as ADLs, ambulation, and vital signs.
  • Perform approved advanced technical skills as required, such as phlebotomy, simple treatments, dressing changes, oral suctioning, and discontinuing Foley catheters.
  • Perform clerical functions including data retrieval, maintaining unit supplies, and transporting supplies/patient medications.
  • Work in a fast-paced environment requiring multi-tasking and effective communication.
  • Perform under stressful conditions with a compassionate, respectful, and service mindset.
  • Take direction from a variety of care providers.
  • Partner with the healthcare team, patient, and family to provide excellence in patient experience.
  • Support an environment of healing or wellness for patients.
  • Follow the plan of care as assigned or instructed by a licensed professional.
  • Engage the patient and family in compliance with the treatment plan.
  • Contribute to the plan of care or treatment based on assigned tasks and patient/family input.
  • Implement all safety interventions to minimize risk.
  • Protect patient rights and dignity.
  • Maintain confidentiality adhering to HIPAA regulations.
  • Participate in hourly patient rounding.
  • Answer telephone and call lights promptly and professionally.
  • Resolve patient concerns or escalate to the charge nurse.
  • Proactively offer assistance and comfort measures to patients and families.
  • Present self in a positive and professional manner.
  • Provide patient care needs such as hygiene, nutrition, elimination, and physical comfort.
  • Perform additional advanced skills based on unit specific requirements (e.g., phlebotomy, EKG, removal of Foley catheters and IVs).
  • Assist RN and other members of the healthcare team with care delivery as assigned.
  • Assist in direct patient admission.
  • Monitor data entry for key patient safety information.
  • Participate in charge reconciliation.
  • Perform audits as requested.
  • Support initiatives of the healthcare team.
  • Participate in unit performance improvement initiatives.
  • Support professional nursing practice.
  • Transport unit supplies, patient care supplies, medications, and equipment as assigned.
  • Practice accountability.
  • Use effective written and oral communication skills with patients, staff, physicians, family, and visitors.
  • Use safety communication techniques.
  • Communicate completion of assignment/duties to RN or Charge Nurse.
  • Document completion of patient care in EMR.
  • Communicate patient care concerns/problems/emergencies to the RN or Charge Nurse in a timely manner.
  • Use positive and respectful communication with all members of the healthcare team, patients, and families.
  • Maintain equipment (e.g., blood pressure machines, glucometers).
  • Sequester, tag, and report malfunctioning devices.
  • Communicate with patients and update white boards with information.
  • Act as unit expert regarding patient care documentation tools during downtime.
  • Provide a safe environment for patient care.
  • Consistent use of proper hand hygiene.
  • Keep patient rooms and unit areas clean and free of clutter.
  • Follow appropriate PPE/Infection control policies.
  • Reinforce the importance of universal precautions with patients and families.
  • Utilize proper body mechanics and patient safe handling devices.
  • Perform environment of care rounds.
  • Comply with all Wellstar Health System policies, standards of work, and code of conduct.

Requirements

  • Wellstar minimum age of hire is 16.
