About the Role

This posting is for a replacement position within our organization. The ideal candidate will provide evidenced based practices for individual, family, and group counselling sessions, manage their own caseload including clinical documentation, facilitate all plans of care, and ensure successful client transitions back to the community within 3 to 6 months of admission. The role requires administering standardized tools and utilizing the results in treatment planning, demonstrating flexibility, and self-motivation. Excellent organizational, communication, and interpersonal skills are essential, as is the ability to work effectively within a dynamic, multidisciplinary team environment.

Responsibilities

Conduct bio-psycho-social assessments, risk assessments, and provide evidenced based regular individual, family, and group therapy sessions with outpatient, day treatment and Live in Treatment clients.
Utilize modalities including Trauma Focused Cognitive Behavioral Therapy, Dialectical Behavioral Therapy, and Emotion Focused Family Therapy.
Attend meetings in office, community and treatment residences including school meetings, case conferences, treatment planning meetings, and psychology/psychiatric appointments.
Attend administrative and clinic team meetings and trainings.
Schedule, facilitate, and lead all Plan of Care meetings, ensuring interdisciplinary members complete their sections appropriately and on time.
Provide full Plan of Care packages to the treatment team and caregivers 3 days prior to the Plan of Care meeting, and obtain all signatures at the conclusion of the meeting.
Meet with Live in Treatment Supervisors and Day Treatment team regularly to coordinate each client’s individual treatment plans.
Set discharge dates within 3 to 6 months of admission and ensure clients are transitioned safely back to the community within that timeframe.
Complete required documentation including assessments, standardized tools, session notes, treatment plans, Serious Occurrence Reports, and disclosures to CAS s in a timely manner.
Liaise with clients and their parents/guardians as well as other professionals such as school personnel, probation workers, and Children’s Aid workers.
Provide intensive individual and family counselling sessions, transition/discharge support, and outreach services to ensure coordination and delivery of services to meet the needs of clients.
Attend weekly or biweekly individual supervision with Clinical Supervisor.
Be available to work 2-3 evenings per week.
Perform other duties as assigned.

Requirements

A Masters of Social Work (MSW) or equivalent education is required.
Minimum 2 years of relevant post qualification experience.
Membership in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) or the College of Registered Psychotherapist of Ontario (CRPO).
Clinical experience in providing bio-psycho-social assessments, risk assessments, and evidenced based individual and family therapy to children, adolescents and/or their families.
Excellent verbal and written communication skills.
Experience with using a variety of evidence-based/informed treatment approaches such as TF-CBT, DBT, EFFT, and/or McMaster Approach to Family Therapy.
Evening availability required.
