About the Role

The Discharge Planner will work within a fast-paced environment as a collaborative team member of an interdisciplinary team. The primary goal is to support and enable patients and families to make timely transitions through the hospital system, aiming for an exceptional patient and family experience. The role involves understanding hospital flow and capacity challenges to ensure patients receive the right care at the right time.

Responsibilities

  • Formulate, facilitate, and coordinate discharge plans focused on patient and family needs and goals.
  • Employ critical thinking and creative problem-solving for all discharge planning, including complex situations.
  • Provide in-depth discharge-focused assessments and collaboratively create discharge plans with the team, patient, and family.
  • Support, educate, and guide the implementation of discharge plans.
  • Facilitate timely transitions to the community by identifying and addressing safety gaps and barriers with internal and external partners.
  • Assess risk and need for psychosocial intervention.
  • Provide psychosocial assessments to identify the need for and provision of clinical social work services or referral to Social Work.

Requirements

  • Bachelors degree in Social Work, Nursing or a Health Discipline is required.
  • Masters degree is preferred.
  • Current registration and membership with a Regulated College.
  • 2-5 years clinical experience in an acute care hospital setting.
  • Experience in Discharge planning is essential.
  • Demonstrated ability to conduct psychosocial assessments and counselling.
  • Demonstrated ability to assess risk and need for referral to Social Work for psychosocial assessment and intervention.
  • Demonstrated excellence in interview skills and ability.
  • Current knowledge of community resources and initiative to proactively search for new resources.
  • Experience with legislation that governs Public Hospitals (Public Hospitals Act, Substitute Decisions Act, Consent to Treatment Act, etc.).
  • Extensive knowledge on how to effectively serve an increasingly aging and diverse population with complex medical, functional, cognitive and psychosocial needs.
  • Comprehensive knowledge of the impact of social determinants of health on discharge planning, hospital LOS, Patient Flow and hospital system capacity.
  • Expert communication, collaboration, organizational and problem-solving skills.
  • Ability to work independently as well as with an interdisciplinary team.
  • Demonstrated ability to set priorities.
  • Skilled working with a broad range of client groups and their significant supports.
