About the Role

We are looking for a dynamic Youth Assertive Community Treatment (ACT) Licensed Mental Health Professional passionate about working with youth ages 10–21 with acute behavioral health needs. The Youth ACT Team addresses the needs of children/youth at risk of entering, or returning home from, high-intensity services through a multidisciplinary team approach. Services are delivered in the home or community, aiming to keep youth stabilized in their homes, schools, and communities.

Responsibilities

Conduct assessments and provide person-centered, trauma-informed, and culturally humble treatment, including crisis intervention.
Participate in daily clinical case conferences, staff meetings, and other program-related meetings, including coordination with service providers, schools, and family members.
Ensure timely and thorough completion of documentation in electronic health records and case files.
Participate in a weekly on-call rotation, at least once per month.
Support team collaboration and communication to ensure quality care delivery.
Engage in professional development and training as required.
Participate in quality improvement initiatives and agency-wide projects.
Flexibly provide coverage and support to team members as needed to maintain continuity of care.

Strong motivation, organizational skills, and openness to adapting for continuous improvement.
Excellent writing and documentation skills.
Proficient in using Electronic Health Records and computer systems.
Experience working with youth and families in both individual and group settings.
Familiarity with the Westchester County System of Services is a plus.

$80,000 (LMSW/LCSW/LMHC/LMFT/LCAT)
$75,000 (LP only)
