About the Role
Provides support and assistance to patients and their families in planning their needs and arranging services. Coordinates services, facilitates discharge and access to services, and promotes continuity of service by connecting the Health System, community, and other organizations for integrated care.
Responsibilities
- Coordinates services.
- Facilitates discharge and access to services.
- Promotes continuity of service.
- Participate in rounds to facilitate an interdisciplinary approach to hospital care.
- Complete assessments for mental health when needed.
- Complete assessments for the Birthing Center as needed.
- Complete safety planning with patients as needed.
- Provides linkages and referrals to community resources as needed.
Requirements
- Demonstrates skill and knowledge of advanced level social work practice.
- Demonstrates the ability to work independently and exhibits flexibility in decision making and prioritizing assignments.
- Demonstrates the ability to develop and maintain collaborative working relationships with all members of the team and community agencies.
- Shows ability to use social work theory and practice models to guide care plans of patients.
- Demonstrates skill in proactively resolving problems, recommending, and implementing continuous quality improvement.

Experience Level
- 2 years’ experience in the social work field
- Hospital social work experience preferred
Education Level
- Master’s in social work from an accredited institution
- LMSW preferred
- MSW with NY state licensure in 12 months accepted
Salary/Compensations
- $30.00 - $42.00/hour
