About the Role

Plan, direct, and coordinate outreach activities to raise program awareness and maintain community relationships. Complete all applications for the Local Department of Social Services, including interviewing consumers, collecting documentation, data input, enrollment, consumer education, and follow-up.

Responsibilities

  • Attend all trainings required by the Local Departments of Social Services and New York State Department of Health.
  • Attend additional trainings specific to the Aged, Blind and Disabled population.
  • Conduct and participate in outreach events to strengthen community rapport and provide education on government-sponsored health insurance.
  • Travel to partner locations to maintain relationships.
  • Provide referrals to other agencies, if applicable.
  • Collect and report data to Director and Associate Director.
  • Report all issues and concerns to Director and/or Associate Director for resolution.
  • Maintain ethical standards, including HIPAA.
  • Assist the Aged, Blind, and Disabled (non-MAGI) population by inputting required household data on applications and accompanying documents.
  • Collect and copy required documents.
  • Explain the submission and approval process.
  • Provide guidance on the Medicare application process.
  • Educate consumers on changes in circumstances affecting eligibility and reporting requirements.
  • Assist the non-MAGI population with renewal completion.
  • Travel to meet consumers as necessary.

Requirements

  • Ability to handle confidential information ethically and professionally.
  • Ability to provide exceptional customer service to all clients.
  • Must be a collaborative team-player willing to provide assistance and perform various tasks as needed.
  • Must be willing to travel throughout NYC, Rockland, and Westchester Counties.
