About the Role
Elara Caring is seeking a dedicated Clinical Team Manager to lead and mentor a team of clinical field staff, ensuring exceptional patient care and team performance. This role involves fostering a culture of compassion, ensuring regulatory compliance, and collaborating with interdisciplinary teams to achieve positive patient outcomes.
Responsibilities
- Lead, mentor, and support a team of clinical field staff, including RNs and LPNs/LVNs.
- Recruit, onboard, train, and develop clinical staff.
- Foster a culture that reflects Elara Caring's mission, values, and commitment to delivering compassionate, high-quality care.
- Ensure compliance with all clinical, regulatory, HIPAA, and organizational policies.
- Review, approve, and monitor clinical documentation.
- Collaborate with interdisciplinary care teams, physicians, patients, caregivers, and families to coordinate care plans.
- Monitor physician orders and documentation for timely completion and compliance.
- Oversee clinical scheduling to ensure authorized services are delivered accurately and efficiently.
- Review productivity, billing, and utilization reports to identify opportunities for improvement.
- Partner with clinicians to resolve scheduling, payroll, documentation, and workflow challenges.
- Ensure accurate billing practices, coding, EVV, and timekeeping documentation.
- Monitor service delivery and reporting processes for accurate documentation, delivery, and billing.
- Investigate and resolve clinical concerns, patient care issues, employee concerns, and service-related challenges.
- Manage client incidents, infection control events, and patient complaints.
- Stay current on industry regulations, accreditation standards, and program requirements.
- Support state, federal, payer, and internal audits.
- Perform nursing visits as needed.
- Participate in on-call responsibilities.
- Contribute to special projects, process improvements, and other initiatives.
Requirements
- Associates degree in Nursing, Social Work, Healthcare Administration, or related field required.
- Bachelor’s degree preferred.
- Minimum of 3+ years of active experience as a RN.
- 1+ year of supervisory home care and administrative experience preferred.
- Knowledge of Medicare/Medicaid, home care benefits, policies, and procedures preferred.
- Strong understanding of state regulatory requirements for HCBS/PCS.
- Experience in incident management, investigations, and regulatory compliance.
- Registered Nurse licensure/certification required.
- Strong communication and interpersonal skills.
- Reliable personal transportation to perform job duties.
- Report to the Clinical Administrator or Director of Nursing.
