About the Role

Responsibilities
- Perform basic and specialized nursing care for patients in their homes.
- Implement medical orders (SGB V) and nursing measures (SGB XI) accurately.
- Observe, document, and assess patient health status, focusing on individual needs.
- Collaborate closely with doctors, relatives, and the team to personalize care.
- Guide and support nursing assistants and contribute to improving nursing quality.
- Actively participate in process improvements and suggest enhancements.
Requirements
- Completed training as a registered nurse, geriatric nurse, or general nurse (m/f/d).
- Enjoyment of independent, self-directed work and making informed professional decisions.
- High standards for personal work quality, even without direct supervision.
- Empathy and genuine interest in patients.
- Teamwork skills and willingness to take on responsibilities beyond individual assignments.
- Valid driver's license Class B for fieldwork in Berlin.
- Ideally, prior experience in outpatient care; motivated individuals from inpatient settings are also welcome.
