About the Role

Change Grow Live is seeking a Homeless Recovery Coordinator for their Tower Hamlets team to support service users with drug issues throughout their treatment and recovery journey. The role involves assessing and engaging service users to ensure they receive the right intervention at the right time.

Responsibilities

  • Work closely with entrenched rough sleepers, encouraging engagement with services and providing rapid assistance.
  • Connect individuals with drug and alcohol support, mental health services, and other healthcare provisions.
  • Oversee service users’ treatment journeys, assist in hospital discharge planning, and collaborate with housing providers to secure appropriate accommodation.
  • Act as the key link between CGL services, rough sleeping partners, hostels, shelters, the local council, and other support agencies to coordinate care and maximise opportunities for recovery.
  • Maintain accurate records, attend case conferences, and contribute to partner tasking meetings to ensure service users receive consistent and effective support.

Requirements

  • Have a strong understanding of substance misuse issues and experience of working within a similar field.
  • Be an excellent communicator, both verbal and written with good IT skills.
  • Proactive and able to work on own initiative with excellent time management and prioritising skills.
  • Have a good working knowledge of mental health interventions, services, and good practice.
  • Be a supportive team player with strong interpersonal skills with the ability to work in partnership with a wide range of professionals, agencies, and internal/external stakeholders.
  • Ability to manage change successfully in a way that prioritises the needs of service users.
  • Understand the importance of information governance processes and commit to follow and apply all necessary safeguards.
  • Seek out learning opportunities to improve and broaden your professional knowledge and skills and to contribute and oversee the learning and development of others.

  • £28,557.79 - £32,802.41 (pro rata if part time)
  • Inner London Weighting (£4,236.47)
