About the Role

The Commonwealth Home Support Program (CHSP) provides entry-level support for older people living in the community through a wellness and reablement model, delivering Community Nursing, Occupational Therapy and Podiatry. This role involves providing meaningful, evidence-based Occupational Therapy services to community clients, coordinating care, supporting patient recovery, and contributing to quality improvement initiatives.

Responsibilities

Provide meaningful, evidence-based Occupational Therapy services to community clients
Coordinate care by liaising with community services, healthcare providers, and support networks to facilitate seamless referrals and continuity of care
Support patient recovery and optimise patient flow through tailored interventions and effective discharge planning
Contribute to quality improvement initiatives, service development, and ongoing professional development

Requirements

Current unrestricted Class C Driver's Licence
Tertiary Qualification in Occupational Therapy and AHPRA Occupational Therapy registration
Minimum 2 years experience working as an occupational Therapist in a community setting
Demonstrated clinical assessment skills, skill in home modification and equipment prescription, and an ability to provide evidence based interventions that ensure safety and maximize independence at home
Demonstrated postgraduate community and rehabilitation experience (e.g. neurological and orthopaedic), including the management of elderly clients with cognitive impairment
Committed to professional development and quality improvement, with strong communication, interpersonal, and computer skills
