About the Role

Rainbow Centre (RC) aims to improve the quality of life for persons with disabilities by providing quality services that support and empower individuals, their families, and communities. The Social Worker will provide high-quality case management, acting as a liaison between clients, healthcare providers, and community resources to coordinate care and advocate for clients' needs.

Responsibilities

• Assess clients' physical, mental, emotional, and social needs through observation and interviews, even in complex cases
• Set goals using standardized and non-standardized assessments
• Plan, implement, and adjust care plans based on clients' goals
• Work with other professionals to create integrated care plans
• Provide individual and group interventions as part of a transdisciplinary team
• Coordinate services and track client progress
• Manage caseload based on priorities, waiting lists, and outcomes
• Develop community programs suited to client needs
• Provide crisis intervention and short-term counseling when needed
• Complete case notes and reports as required
• Advocate for clients within healthcare and social service systems
• Help clients access services and overcome barriers
• Join multidisciplinary meetings to review and adjust care plans
• Reflect on and evaluate own practice regularly
• Stay updated and compliant with relevant laws, policies, and professional codes

Requirements

• Experience working in a similar setting will be an advantage

Education Level

• Recognised degree in Social Work
