About the Role
IOA is revolutionizing healthcare models to enable aging in place, transforming lives and communities while saving healthcare systems significant costs. With over 23 programs, we support seniors' health, well-being, independence, and community participation.
Responsibilities
- Maintain regular contact with assigned clients to ensure referred services are delivered.
- Complete referrals and follow-up calls for ongoing connectivity to healthcare services.
- Conduct outreach and engagement activities for potential clients in various settings.
- Educate clients on healthcare and mental health systems in a culturally appropriate manner.
- Conduct home visits, hospital visits, skilled nursing facility visits, and escort clients to appointments.
- Assist in completing documentation for Medi-Cal waiver participation.
- Identify, arrange, and monitor community services based on knowledge of entitlement programs.
- Establish and maintain professional relationships with clients and their support networks.
- Document client-involved activity and identified problems via progress notes within 24-48 hours.
- Maintain required paperwork and charting systems for continuity of care.
- Establish and maintain communication with community providers, sharing client care information while maintaining confidentiality.
- Collaborate with the interdisciplinary team to support clients' community living.
- Attend and participate in team and program meetings.
- Utilize supervision appropriately and provide updates on client activity.
- Understand and apply regulatory and procedural requirements of the Institute on Aging.
- Attend continuing education and training to enhance knowledge and skills.
- Perform other reasonably related responsibilities as assigned.
Requirements
- At least 2 years of experience working as a Community Health Worker preferred.
- Lived experience navigating the healthcare system with a disability or complex medical/behavioral health condition.
- Familiarity and experience with the cultural and geographic demographics of the population served.
- Experience with and understanding of the medical and psychosocial problems of functionally impaired adults and older adults.
- Experience working with individuals with mental and/or behavioral health diagnoses and substance abuse disorders highly desired.
- Detail-oriented with good problem-solving skills and the ability to prioritize multiple tasks.
- Computer literacy required.
