About the Role
This position provides comprehensive case management to Transitional Aged Youth (TAY) clients utilizing Urgent Accommodation Vouchers for temporary hotel stays. The primary objective is to assess client needs, develop service plans, and facilitate placement into shelter programs, aiming for a smooth transition within their 14-day hotel stay.
Responsibilities
- Provide direct client services including individual peer counseling, case management, advocacy, and accompaniment.
- Conduct new client intake within 24 hours of their hotel stay.
- Enter client information into the ONE database within 48 hours.
- Ensure clients meet with Coordinated Entry within 72 hours of intake and monitor progress towards personal goals.
- Create individualized service plans with prioritized key issues for each client.
- Collaborate with service providers to coordinate care and achieve client service plan goals.
- Assist clients in maintaining stability by offering support and referrals to services.
- Problem-solve with clients to determine the most effective assistance methods.
- Facilitate meetings and support groups as necessary.
- Develop client exit surveys and case summaries for outcome tracking and program analysis.
- Maintain accurate and timely electronic client records in Salesforce, individual case files, daily logs, and service statistics.
- Participate in regular staff and case management meetings.
- Perform other assigned duties.
Requirements
- BA degree in social services, psychology, or a related field; equivalent work experience may be accepted.
- Two years of experience providing services to the chronically homeless is preferred; comparable experience will be considered.
- Above-average proficiency in reading, writing, and speaking English.
- Demonstrated skills in counseling, case management, and client advocacy.
- Familiarity with Harm Reduction, Motivational Interviewing, and OARS philosophies and approaches.
- Strong interpersonal, verbal, and written communication skills.
- Understanding of homeless issues, particularly concerning race, class, gender, and sexual identity.
- Understanding of the coordinated entry system.
- Experience and comfort working with diverse communities, including LGBTQQ, youth, immigrant, low-income populations, and communities of color.
- Proficiency in MS Office Suite (Word, Outlook, Excel).
- Working knowledge of community resources and familiarity with SF homeless shelter systems and services.
- Demonstrated commitment to open, direct, and non-violent communication.
- Ability to de-escalate crisis situations and make decisions under pressure.
- Proficient reporting skills, including data entry, Excel, and Word processing.
- Strong initiative and ability to work independently and cooperatively.
- Ability to maintain professional boundaries with clients, volunteers, and staff.

- COVID vaccination and negative TB test result required.
- Background check completed upon job offer acceptance.
