About the Role
The Care Coordinator will integrate, oversee, and monitor physical and behavioral health needs for adults with severe mental illness, promoting a culture of inclusion and belonging.
Responsibilities
- Integrate, oversee, and monitor all aspects of the physical and behavioral health needs for adults experiencing severe mental illness.
- Complete individual assessments and develop a person-centered plan of care.
- Coordinate referrals and linkage to community services.
- Collaborate with other community service agencies and health care providers.
- Engage in in-person home and community-based activities, providing transportation to members using agency vehicles.
- Promote a culture of inclusion and belonging.
Requirements
- BA/BS in human services required.
- Minimum of 2 years human services experience with individuals experiencing mental illness, substance use disorder, or developmental disabilities.
- 1 year of in-person case management experience preferred (defined as experience in assessment, care planning, knowledge of benefit entitlements, linkage to community providers and services).
- Demonstrated ability to function as a member of an interdisciplinary team.
- Valid and insurable driver's license.
- Must have a personal vehicle to use for business purposes when not transporting clients.
