About the Role

Provide direct care in a day program setting to participants with special needs to enable their highest quality of life in terms of their physical, emotional and mental health while adhering to CCARC policies and procedures.

Responsibilities

  • Provide and receive effective, respectful, appropriate, and courteous verbal and written communications to and from specific teams related to the care of participants.
  • Develop, implement, and maintain accurate written assessment records, annual reviews, biannual or quarterly reviews, and Community or Vocational Service plans.
  • Provide essential physical and emotional care to participants, including assistance with personal care, hygiene, and treatments.
  • Administer medications and record vital signs as needed.
  • Educate participants in personal hygiene, grooming, and behavioral conduct.
  • Coordinate and lead social and recreational activities, including safe transportation of participants.
  • Maintain cleanliness of program spaces.
  • Monitor individuals at mealtime according to food consistency guidelines.
  • Ensure the safety of all individuals, self, and others.
  • Ensure the proper use of equipment, facilities, and vehicles, and report all incidents.
  • Demonstrate knowledge of emergency procedures and respond appropriately.
  • Maintain good daily attendance.
  • Attend and participate in mandatory training, in-services, staff meetings, and conferences.
  • Train, supervise, and support individuals to sustain employment, including adapting equipment/activities.
  • Evaluate individual appropriateness, desires, strengths, and abilities for employment.
  • Act as a primary liaison between employment or volunteer sites and CCARC.
  • Demonstrate adherence to universal precautions, OSHA standards, and infection control policies.
  • Collaborate to resolve conflicts, obstacles, and challenges, seeking guidance from management and team members.
  • Assist in training and support of all substitute staff.
  • Review and maintain information in master files.
  • Maintain good daily attendance (days of absence, illness, tardiness).
  • Attend and participate in mandatory training, in-services, staff meetings, and conferences as required.

Requirements

  • Must have a valid Driver's License.
  • Must be insurable with Agency carrier.
  • Must have a vehicle for use and maintain adequate insurance.
  • Must complete the Agency’s Employee Training.
  • Must be willing to become trained in CPR, First Aid, PMT, and DDS Medication Administration, if not already.
  • Ability to follow oral and written instructions.
  • Experience with Microsoft products, use of email, and Therap preferred.
