About the Role

Provide individual, group and family psychotherapy, supportive counseling, crisis intervention and care coordination for an Article 31 clinic within a Federally Qualified Health Center.

Responsibilities

  • Screen and assess clients for admission into the Article 31 Mental Health Clinic in collaboration with a psychiatrist/psychiatric nurse practitioner during initial visits.
  • Utilize evidence-based tools (PHQ-9, GAD-7, AUDIT, DAST, etc.) and DSM-5 to formulate clinical diagnoses.
  • Hold a caseload of 70 patients, with 10-12 sessions daily.
  • Establish treatment goals and objectives for and with clients as part of the interdisciplinary care planning process.
  • Serve as the primary psychotherapist for assigned clients, documenting all sessions and communications.
  • Participate in on-site crisis response, including rapid assessment, de-escalation, and coordination with multidisciplinary teams.
  • Coordinate and facilitate case conferences for providers across agencies serving mutual clients.
  • Ensure the integrity and confidentiality of all patient information in the EHR.
  • Complete timely, accurate, and comprehensive clinical documentation, including progress notes (72 hours), assessments, treatment plans, and crisis interventions.

Requirements

  • Masters in Social Work plus LMSW/LCSW with at least two years’ experience providing psychotherapy.
  • Experience providing psychotherapy in an Article 31 or similar outpatient clinic required.
  • Experience working with individuals with a history of substance use, mental illness, chronic homelessness and/or HIV/AIDS and other chronic health conditions required.
  • Experience working with late adolescents, adults, and families desired.
  • Experience facilitating therapy groups desired.
  • Candidates already credentialed with insurance panels preferred.
  • Bilingual in Spanish is a plus.
