About the Role
Constellation Health Services is seeking a Physical Therapist Assistant (PTA) to provide physical therapy services to patients in their homes within the North Suffolk area. This field-based role operates under the supervision of a Registered Physical Therapist and focuses on delivering high-quality patient care with compassion and respect.
Responsibilities
- Provides skilled Physical Therapy services.
- Initiates appropriate preventive and rehabilitative physical therapy procedures.
- Teaches and supervises the patient and family in the home environment.
- Coordinates physical therapy with all other professional services involved in patient care.
- Provides direction and supervision to the home health aide regarding simple physical therapy exercises and techniques.
- Confers with the registered physical therapist at least every thirty (30) days, documenting the conference and reviewing the care plan.
- Initiates, reviews, and implements the PT Care Plan for each patient, revising when indicated.
- Observes, evaluates, and reports on patient’s physical and emotional status to team members and physician.
- Provides patient progress reports to attending physician.
- Maintains clinical records for each patient, including observations, treatments, responses, teaching activities, and changes in patient status.
Requirements
- Graduate from an accredited physical therapy assistant educational program.
- Licensed to practice as a Physical Therapist Assistant - PTA in the state of employment.
- Must meet all requirements for regulatory compliance in good standing.
- Has a minimum of one year of clinical experience.
- Previous home care experience preferred.
- Have reliable transportation and be willing to commute.
- Basic knowledge of iOS operating system.
