Responsibilities
- Performing routine and follow-up medical evaluations
- Managing chronic and acute medical conditions
- Collaborating with physicians and interdisciplinary care teams
- Developing individualized treatment plans
- Reviewing labs, diagnostics, and medications
- Communicating with patients, families, and nursing staff
- Helping to prevent unnecessary hospitalizations
- Focusing on delivering compassionate, patient-centered care
- Completing documentation with the support of medical scribes
Requirements
- Active New York State Nurse Practitioner License
- DEA Registration (or ability to obtain)
- Strong clinical assessment and communication skills
- Experience in Family Medicine, Internal Medicine, Geriatrics, Skilled Nursing, Rehabilitation, Long-Term Care, Sub-Acute Care
