About the Role
Liberty Home Care is seeking a motivated Registered Nurse to join our Licensed Home Care team as an RN Field Supervisor.
Responsibilities
- Provide assessment of newly referred patients and assess health needs in a home care setting.
- Collaborate with patients to create a patient-centered plan of care.
- Continuously reassess patient needs for services and discuss any changes in the plan of treatment with the patient's physician or family.
- Prepare written documentation and summaries of patient progress for the continuation of home health services.
- Provide supervision and training of PCA/HHA staff.
- Assist with the on-call rotation (stipend provided).
Requirements
- NYS Licensed Registered Professional Nurse.
- Minimum of Associate's Degree required.
- One year of Nursing experience required.
- Home care experience is a plus.
