About the Role

The hospice RN provides comprehensive nursing care to patients in homecare and facility settings, including admissions, ongoing assessments, symptom management, and psychosocial support. The role involves educating caregivers and facility staff, coordinating patient care using a case-management approach, and maintaining accurate documentation. The nurse collaborates with physicians and interdisciplinary teams to ensure high-quality care and smooth transitions.

Responsibilities

  • Provides professional nursing care for hospice patients in homecare and assigned facility settings.
  • Performs initial and ongoing assessment of the impact of the terminal diagnosis on patient needs.
  • Performs comprehensive nursing assessments, including vital signs and pain assessment.
  • Triages patient care concerns and determines appropriate interventions.
  • Initiates preventive, comfort-focused, and rehabilitative nursing procedures.
  • Develops, maintains, and updates individualized plans of care.
  • Consults with and educates patients, caregivers, and facility staff.
  • Provides specialized training to staff and/or caregivers.
  • Performs ongoing evaluation of patient and family response to care.
  • Provides psychosocial support to patients and families.
  • Provides preparation and support for the patient’s death, pronounces death, and provides postmortem care.
  • May provide nursing care to pediatric patients as needed.
  • Utilizes proper infection control and bag technique.
  • Reports adverse events to the Team Leader or Quality Department.
  • Performs point of service documentation and completes all documentation according to agency protocol.
  • Complies with minimum weekly visit requirements.
  • Conducts admissions visits for new patients in assigned facilities.
  • Provides ongoing education in assigned facilities.
  • Coordinates nursing, personal, and homemaker care for hospice patients.
  • Attends IDT meetings and coordinates patient and family care.
  • Informs the physician and other team members of changes in patient condition.
  • Determines the scope, frequency, and type of services needed.
  • Coordinates personal care services, durable medical equipment, medications, and medical supplies.
  • Uses a case management approach, referring to other disciplines as needed.
  • Provides supervision of care delivered by CNAs, LVNs, and homemakers.
  • Documents patient assessments, contacts, physician orders, and clinical/progress notes.
  • Ensures hospital and facility charts include documentation after each patient visit.
  • Acts as a liaison between patients, physicians, hospital staff, and other care team members.
  • Collaborates with Hospice Austin and hospital/facility staff regarding discharge planning.
  • Evaluates personal needs for support and self-care.
  • Recognizes and maintains professional boundaries.
  • Assists in developing and implementing discharge plans.
  • Conducts admissions visits for new patients in assigned facilities, coordinating with facility staff.
  • Provides ongoing hospice education in assigned facilities.
  • Conducts other duties as assigned.

Requirements

  • Graduation from an approved school of professional nursing.
  • Unrestricted license as a Registered Nurse by the Texas Board of Nurse Examiners.
  • Valid Texas driver’s license.
  • Auto liability insurance.
  • Safe driving record.
  • CPR certification.
