About the Role
The Resident Care Coordinator assists with coordinating resident care through completion of the MDS, rounding with physicians, care planning, and providing support to direct care staff. This role is crucial for ensuring comprehensive resident care and accurate financial reimbursement.
Responsibilities
- Serve as the wound care coordinator.
- Coordinate resident care through completion of the MDS.
- Serve as the Restorative Program Coordinator.
- Oversee care plan meetings.
- Participate in physician rounds and assist direct care staff.
- Perform other duties as requested.
Requirements
- Two-year degree in nursing required.
- Must hold and maintain Wisconsin Registered Nursing license.
- Wound care certification, RAC certification preferred.
- MDS certification a plus.
- Current CPR certification.
- 6+ months of related work experience required.

- Intermediate proficiency with computers.
- Exceptional clinical skills.
- Strong oral and written communication skills.
