About the Role

FOX Rehabilitation is seeking passionate, licensed Physical Therapists to deliver exceptional one-on-one care to older adults in their homes or senior living communities. This role emphasizes improving patient mobility, independence, and quality of life through a Geriatric House Calls™ model, offering autonomy, flexibility, and meaningful patient outcomes. Clinicians can also incorporate telehealth and virtual care.

Responsibilities

Provide one-on-one physical therapy to older adults in private residences, Senior Living Communities, Independent Living, Assisted Living, and Memory Care settings.
Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
Deliver therapy sessions focused on patient wellness, longevity, mobility, and functional independence.
Document effectively within FOX’s EMR system according to regulatory standards.
Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and Fitness Specialists.
Educate patients, caregivers, and senior living staff on safety, function, and aging in place.
Provide telehealth/virtual care to enhance access and flexibility.

Requirements

Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
Degree from an accredited Physical Therapy program.
Basic computer and EMR proficiency.
Current CPR certification.
Ability to meet physical demands of in-home and community-based care, including lifting, performing CPR, assisting with patient transfers, repositioning, standing/walking for extended periods, climbing stairs, performing therapeutic activities, and driving.
Must wear PPE when required.

Salary/Compensations

Full-Time: $75,000 - $115,000 annually
Part-Time and FOX Flex (PRN) will be pro-rated.
