Responsibilities

Deliver person-centred functional and environmental assessments and interventions that empower clients to maintain independence and improve quality of life.
Provide specialised equipment solutions and pressure care strategies.
Manage a wide variety of client presentations across multiple community aged care funding programs including Home Care Packages, Commonwealth Home Support Program, Transitional Care Program and Palliative Care.
Conduct in-home assessments and develop tailored treatment plans to help clients meet their goals, including simple and complex equipment prescription, major and minor home modifications, and client education.
Collaborate within a supportive statewide Occupational Therapy team and work locally with a diverse multidisciplinary team of nurses, care workers and support staff.
Participate in innovative areas such as telehealth, evidence-based group therapies, and acute care services.

Requirements

Current AHPRA registration with unlimited work rights (essential)
Current Australian Driver’s Licence (essential)
Access to a roadworthy, insured vehicle (minimum Third-Party Property) (essential)
Previous experience in an Aged Care or Acute setting (desirable)
Strong communication skills and the ability to build meaningful relationships
Effective time-management and sound clinical problem-solving abilities
Confidence to meet all relevant probity and compliance requirements

Salary/Compensations

$42.15 – $59.04 p/hr (dependent on experience) + Super + Not‑for‑Profit Salary Packaging + Kilometre Reimbursement + Penalties & Allowances
