About the Role

Graham provides life-changing tools and resources for children, young adults, and families facing obstacles caused by poverty, racial injustice, and lack of access to educational opportunities, living wage employment, quality healthcare, and affordable housing. Our Community Mental Health & Wellness Clinic offers evidence-based treatment for children, youth, young adults, and their families aged 5-25, including individual, family, and group therapy, as well as evaluations and psychopharmacology management.

Responsibilities

  • Provide individual, group, family, and collateral treatment and crisis services/interventions that are focused and goal-oriented.
  • Offer information and counseling on the best course of action during sessions.
  • Apply appropriate evidence-based psychotherapeutic interventions to assess, monitor, and ameliorate symptoms, and minimize negative effects of trauma symptoms, behaviors, and environmental stressors.
  • Provide, implement, and coordinate comprehensive initial and ongoing assessments and treatment plans for new intake clients and ongoing clients.
  • Collaborate with clients, collaterals, and multi-disciplinary clinical treatment teams in developing, reviewing, and modifying treatment plans.
  • Monitor and evaluate clients’ progress and modify treatment plans as needed.
  • Consult with other behavioral health, primary care, and specialty care providers as appropriate.
  • Collaborate with outside organizations such as schools, hospitals, higher level of care programs, and other service programs on behalf of client’s care needs.
  • Provide engagement and rapport to foster commitment to therapeutic relationships and regular attendance.
  • Explore motivation, resistance to therapy, and attendance as needed.
  • Maintain electronic records and documentation according to OMH, Medicaid, and Graham standards.
  • Maintain expected levels of productivity and provide quality care.
  • Provide crisis intervention services to de-escalate situations of acute distress and manage reportable incidents.
  • Coordinate the discharge planning process, identify resources, and make necessary referrals and linkages.
  • Be actively self-reflective and aware of how clinical and administrative practice affects clients, families, and communities.
  • Be open and willing to receive constructive feedback.
  • Approach work with an anti-racism lens and be open to continuous learning and professional growth.
  • Participate in clinical meetings, staff meetings, trainings, supervision, and case conferences.
  • Maintain professional behaviors and ethical standards.
  • Uphold the privacy of patient, client, or protected member health information.

Requirements

  • Master’s degree with professional license: LMSW, LCSW, LMHC, LMFT, LCAT.
  • Limited Permit holders eligible.
  • MSW permit holders must obtain a license within 1 year.
  • MHC/MFT/CAT permit holders must obtain a license within 2 years.
  • 1 year of post-masters experience of clinical experience with children/adolescents/families.
  • Ability to work evenings and/or Saturdays.
  • High degree of computer literacy.
  • Demonstrated experience with diverse populations preferred.

  • Graham Windham’s Community Mental Health & Wellness Clinic is an Article 31 OMH program that offers a variety of evidence-based treatment for children, youth, young adults, and their families from the age of 5-25 years of age, based on our comprehensive psychiatric and biopsychosocial evaluations.
  • We offer individual therapy, family therapy, group therapy, psychological and psychiatric evaluations, psychopharmacology management, and support services.
  • Our Harlem clinic offers walk-in hours for immediate mental health assessments.
  • Each treatment plan is created in collaboration with the client based on the needs of the individual client.
  • We also offer bilingual services in Spanish to better support the needs of our communities.
  • We have 2 openings for our 1946 Webster Avenue, Bronx, NY 10457 location and one for our 274 West 145th Street, New York, NY 10039 location.
