About the Role
Provide ongoing support to people during overnight and morning hours, assisting with self-care routines, health, safety, and overall well-being to maintain a high quality of life.
Responsibilities
- Practice and promote the agency’s mission statement in every interaction.
- Support people with self-care routines including toileting, feeding, showering, dressing, and diapering.
- Ensure a safe and clean home environment by assisting with and participating in household chores.
- Follow all treatment plans and utilize adaptive and safety equipment.
- Report and document any events that may hinder the health, safety, and well-being of residents.
- Act as a mandated reporter to the Justice Center for suspected abuse or neglect.
- Adhere to Federal and New York State regulations, agency policy, and procedures.
- Complete and maintain AMAP certification and assist with medication administration.
- Arrive to work on time and adhere to agency policies regarding call-offs and overtime.
- Support people in preparing meals and teaching concepts like diet adherence and portion control.
- Assist people with medication administration.
- Transport people using safe driving practices to appointments or locations.
- Implement and document all treatment plans and agency-related materials.
- Serve as a member of the inter-disciplinary team by attending and participating in meetings.
- Mentor and act as a role model for new Direct Support Staff.
- Perform other duties as requested by supervisor.
Requirements
- Must have a qualified New York State Driver’s license in accordance with agency’s insurance standards.
- Must have successful completion and maintenance of AMAP status.
- Ability to communicate and understand written and oral directives/information.
- Must be an excellent team player with a positive attitude.
- Qualified driver status is required for overnight Direct Support Professionals (11pm – 9am) for morning transportation.
- Subject to successful completion of fingerprint/criminal background check due to regular and substantial unsupervised contact with people receiving services.
