About the Role

The Senior Case Manager ensures the smooth operation and coordination of case management services, meeting program goals and performance outcomes. This role involves providing advocacy services, supervising staff, and participating in an interdisciplinary treatment team to facilitate client recovery and rehabilitation.

Responsibilities

  • Coordinate, guide, and supervise direct reporting staff
  • Provide direct supervision of Case Managers
  • Communicate contract requirements to staff
  • Lead and participate in administrative and staff meetings
  • Troubleshoot problems and make decisions based on program policies and protocols
  • Monitor client progress and document via progress notes
  • Conduct periodic reviews and chart auditing of client files for accuracy and completeness
  • Manage a case portfolio of approximately 25 clients
  • Monitor the therapeutic environment to ensure behavioral guidelines are followed
  • Provide individual counseling and facilitate caseload group meetings
  • Provide psycho-education and counseling on chemical dependency and relapse prevention
  • Administer behavioral interventions as needed
  • Participate in assessment screenings to determine client needs
  • Formulate and implement treatment, transition, and discharge planning
  • Attend case conferences and scheduled meetings

Requirements

  • High School Diploma or equivalent with 5 years of experience supervising Case Managers in transitional/shelter programs
  • Associates Degree in Human Services with 2-3 years of experience supervising Case Managers in transitional/shelter programs
  • Bachelor’s Degree in Human Services with 1-2 years of experience supervising Case Managers in transitional/shelter programs
  • Working knowledge of substance abuse treatment and client self-help modalities
  • Available to work a flexible schedule
  • Experience working with people from diverse racial, ethnic, and socioeconomic backgrounds
